About The Position

The Clinical Lead – Home and Community Care reports to and works under the direction and supervision of the Senior Manager of Primary and Clinical Care, Six Nations Elected Council. The role provides support to the coordination and delivery of culturally appropriate clinical services by the Home and Community Care (HCC) Team. It also provides clinical guidance for trauma-informed care, promoting the harmonization of Western and Traditional Hodi:noshoni:/Rotinonhsion:ni knowledge.

Requirements

  • Master’s Degree in Regulated Health Profession and a member in good standing or eligible for membership with their regulated Health Professional College of Ontario OR Bachelor’s Degree in Regulated Health Profession and a member in good standing or eligible for membership with their Regulated Health Professional College of Ontario OR Registered Nurse with College Diploma and a member in good standing or eligible for membership with the College of Nurses
  • Demonstrated experience working in an inter-disciplinary team.
  • Demonstrated leadership experience.
  • Working knowledge of Regulated Health Professionals Act as well as profession specific regulations and standards.
  • Familiar with Electronic Medical Record systems.
  • Understands the importance of Confidentiality.

Responsibilities

  • Provide support to the coordination and delivery of culturally appropriate clinical services.
  • Provide clinical guidance for the provision of trauma-informed care.
  • Promote the harmonization of Western and Traditional knowledge.
  • Ensure staff adhere to relevant legislation, standards, and regulations for documentation, privacy, confidentiality, scope of practice, and ethics.
  • Support team members in maintaining client records according to regulatory college guidelines and practice standards.
  • Assist Program Managers and Supervisors with onboarding new team members.
  • Support clinical staff in implementing trauma-informed care models.
  • Support staff in developing and implementing culturally appropriate client care plans.
  • Support the implementation of an inter-professional model of care.
  • Coordinate inter-professional training and education for the team.
  • Support intra-professional collaboration and communication.
  • Participate in the Esadatgehs (Quality) Committee and associated quality working groups.
  • Attend Health Leadership Team Meetings.
  • Act as a Super User to support the Electronic Medical Record (EMR) Committee and provide EMR support to staff.
  • Participate in the Clinical Lead Department of Well-Being community of practice.
  • Provide clinical supervision to staff.
  • Attend monthly Clinical Rounds.
  • Conduct regular chart audits.
  • Assist staff in problem-solving, including ethical dilemmas, client concerns, and incident reporting.
  • Utilize culturally appropriate, evidence-informed practice principles.
  • Comply with established safety precautions, infection control procedures, and aseptic and isolation techniques.
  • Ensure actual work performed is documented and placed in the EMR.
  • Assist in the development and maintenance of service standards.
  • Ensure staff safety while performing duties.
  • Attend HCC staff meetings.
  • Employ appropriate communication skills with clients, families, and staff.
  • Attend and chair HCC case conferences and clinical rounds.
  • Follow procedures for reporting hazardous conditions, equipment, and incidents.
  • Ensure privacy is maintained in accordance with PHIPA and departmental policies.
  • Provide appropriate advocacy for clients and families.
  • Communicate regularly with all programs within the Primary and Clinical Care Portfolio.
  • Meet regularly with the Senior Manager of Primary and Clinical Care to provide updates.
  • Plan weekly itinerary and complete monthly work reports and statistics.
  • Follow established reporting procedures.
  • Complete necessary monthly report forms and maintain relevant statistics.
  • Coordinate and schedule clinical activities.
  • Support the program development and evaluation process.
  • Review service policy and procedures manual, work plans, and job descriptions annually.
  • Develop, maintain, and create reports for clinical and quality indicators.
  • Support the Senior Manager in preparing and reviewing quarterly statistics and financial reports.
  • Collaborate with other departmental Clinical Leads and the Professional Practice Office.
  • Perform other job-related duties as may reasonably be required.
  • Support the program in human resource processes.
  • Provide supervision of students and volunteers.
