About The Position

The Therapist/Clinical Case Manager will provide individual therapy, case management, and clinical consultation as part of the Wake County UNC FIT Wellness Program, serving formerly incarcerated clients with serious mental illnesses. In addition to direct care, this role will support the development and supervision of the Community Health Workers (CHW) and contribute to administrative tasks that enhance program function.

Requirements

Experience working with health care systems, mental health and substance use treatment programs, community-based organizations.
Experience with adult care home placement and residential substance use treatment referrals.
Experience with referrals for enhanced psychiatric services (e.g. ACT, CST).
Familiarity with interventions for co-occurring disorders and experience in working with a diverse client population.
Experience within the past 5 years working as part of a multi-disciplinary team, collaboratively practicing with peer support workers, psychiatrists, PCPs, health workers, health educators, and/or other health professionals.
Experience with Comprehensive Clinical Assessment (CCA), treatment planning, conducting interviews/investigations, advising/counseling clients, and participating in health promotion and health education activities.
Excellent organization, follow through, and ability to juggle multiple priorities in fast-paced environment with multiple collaborators
Excellent verbal and written communication skills.
