Requirements

• Master of Social Work (MSW) degree and licensed as either a LCSW or a LMSW in the State of Georgia OR Master’s degree in physical therapy or occupational therapy and licensed in the State of Georgia
• One year clinical experience in Social Work, Care Coordination or Rehabilitation.

Responsibilities

• Conducts preadmission screenings for Glancy Rehabilitation Center
• Provides support for the Intake/Discharge Coordinator as needed
• Serves as a liaison between Glancy Rehabilitation Center and physicians
• Plans and coordinates community events that represent Glancy Rehabilitation Center
• Markets the hospital system's services within the community
• Establishes and maintains effective working relationships with physicians
• Serves as an information source for community awareness of the Glancy Rehabilitation Center and the hospital system
• Participates in budget development and cost saving efforts
