About The Position

The Central Referral Center (CRC) for BJC Home Care Services is where the patient referral process starts. There are Intake Coordinators in each BJC hospital who coordinate services for those patients that need home care. The patients who choose BJC Home Care Services are then channeled through the CRC. The CRC also consists of nurses who take physician office referrals for patients not currently hospitalized. There is someone on-call 24/7 for the CRC to facilitate home care coordination for patients in emergency rooms, etc. This role supports Barnes Jewish Hospital Referrals and works with Hospice patients and their families in the next stage of care.

Requirements

• Associate's Degree
• < 2 years experience
• < 2 years supervisor experience
• RN license
• Registration with the Family Care Safety Registry

• Prior Hospice experience
• Prior Lead experience
• Bachelor's Degree
• 5-10 years experience
• 2-5 years supervisor experience

Responsibilities

• Serves as the lead for the Referral Coordinator's.
• Directs the patient flow process for the Referral Coordinators.
• Coordinates the day-to-day operations in respective areas.
• Assist in training new staff.
• Provides updates to the Intake Manager.
• Promotes and holds employees responsible for ICARE standards.
• Performs complete and correct data entry of necessary information.
• Identifies and resolves problems encountered in the order entry and confirmation processes.
• Handles customer compliments and complaints, updates appropriate areas of patient information in patient records and other documents as needed.
• Assists staff with trouble-shooting unusual accounts and escalated patient concerns.
