About The Position
Join the Case Management Acute Care team at Lovelace Women’s Hospital in Albuquerque, New Mexico, as a rotating-shift, PRN Clinical Case Manager. The role assesses patients’ and families’ social, psychosocial, cultural, environmental, and financial situations. The Clinical Case Manager advocates for patients and families, promotes their rights and dignity, and seeks to involve them in care as appropriate. The position also facilitates acute, rehabilitative, and long-term discharge processes to support an optimal, age-specific continuum of care. Lovelace Women’s Hospital has a 53-bed advanced neonatal intensive care unit, a family care unit, an award-winning breast care center, and the state’s first robotic surgery program recognized as a Center of Excellence in Robotic Surgery by Surgical Review Corporation.
Requirements
- A bachelor’s degree in social work or a related BA/BS, an associate’s degree in nursing, or a nursing diploma.
- For the Registered Nurse pathway: a valid New Mexico nursing license or New Mexico compact license; the compact license must be obtained within 90 days of hire.
- For the Registered Nurse pathway: BLS certification must be obtained within 14 days of hire or transfer into the role and before providing direct patient care.
- The hiring department may also require ACLS and/or PALS.
Nice To Haves
- For the Social Worker pathway: Medical Social Worker or Licensed Clinical Social Worker credentials are preferred.
- A master’s degree in social work or a bachelor’s degree in nursing.
- Two years of medical social worker or RN Case Manager experience.
- CCM or ACM certification.
