About The Position

Pleasant Point Senior Living is seeking a Clinical Care Director to lead clinical operations and resident care services at its CBRF and RCAC in Racine, Wisconsin. This is a clinical leadership role focused on providing exceptional care to seniors and building a strong, resident-centered culture. The director works with residents, families, team members, physicians, pharmacies, and therapy providers to coordinate care and support resident well-being.

The role includes overseeing assessments, service planning, evaluations, regulatory compliance, clinical quality, medication management, documentation, and risk reduction. It also includes developing the clinical team and supporting occupancy through resident assessments and admission evaluations. Requirements

• Minimum 1–3 years of experience in assisted living, senior living, or healthcare.
• Strong clinical assessment, communication, and organizational skills.
• CPR certification as required.
• Passion for serving older adults and supporting team development.

Nice To Haves

• Current LPN license preferred.
• Prior leadership or supervisory experience strongly preferred.

Responsibilities

• Lead and oversee all clinical operations and resident care services.
• Conduct resident assessments, service planning, and ongoing evaluations.
• Ensure compliance with state regulations, resident rights, HIPAA, and infection control standards.
• Recruit, coach, mentor, and develop a high-performing clinical team.
• Partner with residents, families, physicians, pharmacies, and therapy providers to coordinate care.
• Monitor quality outcomes, medication management, clinical documentation, and risk reduction initiatives.
• Support occupancy efforts through resident assessments and admission evaluations.
• Foster a culture of accountability, compassion, and excellence in resident care.
