Requirements

  • Current, active, unrestricted New York State Registered Nurse license.
  • Minimum of 5 years of clinical nursing experience.
  • Experience in managed care, PACE, MLTC, home care, case management, utilization management, population health, or related healthcare settings.
  • Strong clinical judgment, communication, and documentation skills.
  • Ability to travel throughout The Bronx & Westchester

Nice To Haves

  • BSN preferred.
  • Experience utilizing UAS-NY assessments.
  • Knowledge of PACE and Community-Based Long-Term Care programs.
  • Experience working with frail, elderly, or medically complex populations.
  • Bilingual language skills strongly preferred.

Responsibilities

  • Conduct comprehensive in-home clinical assessments utilizing the UAS-NY.
  • Evaluate participant eligibility and ongoing care needs for community-based long-term care services.
  • Perform reassessments and identify changes in condition, functional status, diagnoses, and medication regimens.
  • Develop proposed service plans and recommendations to support participant health and independence.
  • Provide hands-on services in participants’ homes as needed, including, but not limited to, administering medications via appropriate routes, performing wound care, and carrying out other clinical procedures within the scope of practice.
  • Collaborate with Enrollment, Intake, Care Delivery, and IDT teams.
  • Document clinical findings accurately and timely within electronic medical record systems.
  • Participate in quality improvement initiatives and serve as a clinical resource throughout the assessment process.
