About The Position

As part of the CenterLight Health System family, we help older adults access coordinated healthcare services that allow them to remain safely in their homes and avoid unnecessary institutional care. As a Clinical Assessment Manager (RN) – Intake, you will serve as one of the first clinical touchpoints in a participant's journey. Your assessments will help determine eligibility, identify healthcare needs, and connect individuals with services that can significantly improve their quality of life.

Requirements

  • Active, unrestricted New York State Registered Nurse license.
  • Minimum of 1-2 years of clinical nursing experience.
  • Experience in managed care, MLTC, PACE, home care, case management, utilization management, or population health programs.
  • Experience conducting clinical assessments and making independent clinical determinations.
  • Ability to travel throughout The Bronx and Manhattan.

Nice To Haves

  • BSN preferred.
  • UAS-NY experience strongly preferred.
  • Experience supporting participant intake, enrollment, or eligibility determination.
  • Knowledge of PACE and Community-Based Long-Term Care programs.
  • Experience working with frail, elderly, or medically complex populations.
  • Bilingual language skills strongly preferred.
  • Chinesse bilingual

Responsibilities

  • Conduct comprehensive in-home and virtual assessments utilizing the UAS-NY.
  • Evaluate prospective participants for eligibility and appropriateness for PACE services.
  • Assess clinical, functional, psychosocial, and environmental needs.
  • Partner closely with Intake, Business Development, and Care Delivery teams.
  • Educate prospective participants and families regarding PACE services and benefits.
  • Gather required clinical documentation needed for enrollment decisions.
  • Present clinical findings and recommendations that support enrollment decisions and care planning.
  • Ensure compliance with Centers for Medicare & Medicaid Services and NY State Department of Health guidelines.
  • Maintain accurate and timely documentation within electronic medical record systems.
