About The Position

H2 Health Prime Living is seeking a Clinic Director to provide 1:1 patient care within senior communities, specifically at Grand Living at Tamaya in Jacksonville Beach. This full-time role (34+ visits/week) offers a flexible schedule and the opportunity to see all patients within one location. As a Clinic Director, you will partner with residents, community stakeholders, and the administrative team to meet resident needs, manage a caseload, and act as an extension of the communities served. Requirements

• A graduate of an accredited Occupational Therapy Assistant program.
• An active or eligible state license to practice.
• Prior experience is preferred, but new graduates are welcome.

Responsibilities

• Partner closely with residents, community stakeholders, and the administrative team.
• Ensure residents’ needs are consistently met.
• Effectively manage and maintain an appropriate caseload.
• Provide 1:1 patient care.
• Act as an extension of the communities that we serve.
