About The Position

H2 Health Prime Living is seeking a Clinic Director to provide 1:1 patient care in senior communities. This full-time role involves managing a caseload and ensuring residents' needs are met through collaboration with residents, community stakeholders, and the administrative team. Requirements

• A graduate of an accredited Occupational Therapy Assistant program.
• An active or eligible state license to practice.

Responsibilities

• Partner closely with residents, community stakeholders, and the administrative team to ensure residents’ needs are consistently met.
• Effectively manage and maintain an appropriate caseload.
• Provide 1:1 patient care.
• Act as an extension of the communities that we serve.
