About The Position

Rio Grande Rehabilitation & Healthcare Center is looking for a compassionate and dependable Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join their team as a Charge Nurse. This role involves providing exceptional resident care while leading and supporting a dedicated nursing team. The position is full-time with rotating weekends.

Requirements

• Current Colorado LPN or RN license in good standing.
• Strong leadership, communication, and critical thinking skills.
• Compassionate, dependable, and committed to delivering exceptional resident care.

Nice To Haves

• Previous skilled nursing or long-term care experience preferred.

Responsibilities

• Supervise and coordinate nursing care for residents during your shift.
• Administer medications and treatments as prescribed.
• Monitor residents' conditions and promptly communicate changes to physicians and the interdisciplinary team.
• Provide leadership, guidance, and support to CNAs and other nursing staff.
• Ensure accurate and timely documentation.
• Maintain compliance with all state and federal regulations and facility policies.
• Foster a positive, team-oriented environment focused on quality resident care.
