About The Position

Transition Home Healthcare is an elite home health agency serving Houston, Texas, and surrounding areas. The agency is seeking a Certified Occupational Therapist Assistant to support rehabilitation and occupational therapy care for patients in the Southwest, Inner Loop, and Southeast service areas. The role includes implementing care plans, coordinating with the care team, educating patients and families, and maintaining accurate clinical documentation. Full-time and PRN shifts are available.

Requirements

• Certified Occupational Therapist Assistant in the State of Texas.
• Knowledge of Home Health regulations and general healthcare compliance.
• Ability to handle a physical workload.
• Good driving record.

Nice To Haves

• Home Health experience is highly preferred.

Responsibilities

• Facilitate rehabilitation for patients with emotional, physical, or mental disabilities.
• Implement occupational therapy care plans.
• Coordinate care with the entire care team.
• Educate patients and family members.
• Prepare timely, complete, and accurate clinical notes.
• Communicate effectively with patients, caregivers, healthcare providers, office support, and other care team members.
