About The Position

Moments Hospice is hiring a Certified Nursing Assistant (CNA) / Hospice Aide to provide hands-on personal care to hospice patients in their homes, assisted living communities, or care facilities. The CNA helps patients remain comfortable, clean, and cared for while offering reassurance and support to their families.

The role works closely with a dedicated RN Case Manager as part of an interdisciplinary care team. The position is field-based, with a Monday–Friday schedule from 8:30 AM to 5:00 PM. Moments Hospice offers structured training, reasonable caseloads, and opportunities to develop skills. A fleet car is available for business and personal use at minimal cost, with fuel and insurance covered, subject to eligibility.

Requirements

• Active Certified Nursing Assistant (CNA) certification.
• Ability to work in an electronic medical record system.
• Strong written and verbal communication skills, including active listening.
• Customer service, problem-solving, and time-management skills.
• Compassion, patience, and ability to handle emotional situations with care.
• Strong understanding of professional boundaries.
• Ability to lift 50 lbs. and good understanding of body mechanics.
• Valid driver's license and current auto insurance.

Nice To Haves

• Healthcare caregiving experience in home care, assisted living/memory care, skilled nursing, a hospital, or hospice.
• Current CPR certification.

Responsibilities

• Provide personal care with activities of daily living, including bathing, dressing, grooming, toileting, and skin care.
• Assist with lifting, transferring, positioning, range of motion, exercise, and ambulation.
• Assist with dining and nutrition.
• Review and follow each patient's plan of care under the direction of the RN Case Manager.
• Provide comfort and companionship to patients and support to their families.
• Maintain electronic medical records.
