About The Position

Moments Hospice is hiring a full-time, field-based Certified Nursing Assistant (CNA), also referred to as a Hospice Aide. The CNA provides hands-on personal care to hospice patients in private residences, assisted living settings, and care facilities, helping them remain comfortable, clean, and cared for. The role also provides support and companionship to patients’ families.

The CNA works closely with a dedicated RN Case Manager as part of an interdisciplinary care team. The position offers structured training, clear expectations, and reasonable caseloads. The schedule is Monday through Friday, 8:30 AM to 5:00 PM. A company vehicle is available for business and personal use at minimal cost, with fuel and insurance covered.

Requirements

• Active Certified Nursing Assistant (CNA) certification is required.
• Valid driver's license and current auto insurance are required.
• Ability to work in an electronic medical record system.
• Strong written and verbal communication skills, including active listening.
• Excellent customer service, problem-solving, and time management skills.
• Compassion, patience, and ability to handle emotional situations with care.
• Strong understanding of professional boundaries.
• Ability to lift 50 lbs. and a good understanding of body mechanics.

Nice To Haves

• Healthcare caregiving experience in home care, assisted living or memory care, skilled nursing, a hospital, or hospice.
• Current CPR certification.

Responsibilities

• Provide personal care and assistance with activities of daily living, including bathing, dressing, grooming, toileting, and skin care.
• Assist with lifting, transferring, positioning, range of motion, exercise, and ambulation.
• Assist patients with dining and nutrition.
• Review and follow each patient's plan of care under the direction of the RN Case Manager.
• Provide comfort and companionship to patients and support to their families.
• Maintain electronic medical records.
