About The Position

Linwood Meadows Care Center is seeking a Certified Nursing Assistant (CNA) for a Full-Time PM Shift. This role is crucial in providing compassionate, person-centered care in a transitional setting between hospital and home. CNAs are responsible for delivering routine daily nursing care and services to assigned residents, adhering to their individual care plans and facility policies.

Requirements

• Active California Certified Nursing Assistant (CNA) certification required
• Ability to read, write, and communicate effectively in English
• Ability to work in a fast-paced environment with compassion and professionalism
• Strong teamwork and interpersonal skills
• Ability to lift, push, pull, and/or move up to 25 pounds
• Frequent standing, walking, bending, and assisting with resident mobility

Nice To Haves

• CPR certification preferred

Responsibilities

• Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, mobility, and dining
• Measure and record vital signs (TPRs), weights, and intake/output as instructed
• Respond promptly to call lights and ensure residents unable to call for assistance are checked frequently
• Observe and report changes in resident condition, skin integrity, or behavior to the Charge Nurse
• Assist with admissions, transfers, and discharges as directed
• Provide restorative and rehabilitative care under nursing supervision
• Maintain resident rooms, equipment, and personal belongings in a clean and orderly manner
• Follow infection control, safety, and confidentiality policies at all times
• Report accidents, incidents, concerns, or potential violations immediately
• Foster a warm, respectful, and positive environment for residents, families, and coworkers
