About The Position

Moments Hospice is hiring a Certified Nursing Assistant (CNA) to help provide patient comfort and dignity during patients’ final days. The CNA performs personal care and daily living activities, assists with mobility, nutrition, and other aspects of patient care, and works under the direction of a Case Manager RN. The role involves maintaining electronic medical records and reviewing patient care plans. The position is on the road and includes a supportive team environment, reasonable caseloads, on-call support, and structured training. A company vehicle may be available for business and personal use at minimal cost, with fuel and insurance covered.

Requirements

• Certified Nursing Assistant (CNA) license.
• At least 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital, or Hospice.
• Excellent customer service skills.
• Strong written and verbal communication and active listening skills.
• Problem-solving skills.
• Time management skills.
• Ability to work in an electronic medical record system.
• Ability to lift 50 lbs.
• Good understanding of body mechanics.
• Strong understanding of professional boundaries.
• Valid driver's license.
• Valid auto insurance.

Nice To Haves

• Current CPR certification.

Responsibilities

• Perform personal care activities related to daily living, including bathing, dressing, grooming, toileting, and skin care.
• Review patient care plans.
• Assist with lifting and transferring patients.
• Assist with range of motion, positioning, exercise, and ambulation.
• Assist with dining and nutrition.
• Maintain electronic medical records.
• Work under the direction of a Case Manager RN.
