About The Position
Moments Hospice is hiring a Certified Nursing Assistant to support patient comfort and dignity during their final days. The role involves providing personal care and assisting patients with daily living activities while working under the direction of a Case Manager RN. CNAs review care plans, maintain electronic medical records, and collaborate with a dedicated RN Case Manager. The company describes a supportive environment with clear expectations, reasonable caseloads, on-call support, and structured training. Requirements
- Certified Nursing Assistant (CNA) license.
- 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital, or Hospice.
- Excellent customer service skills.
- Strong written and verbal communication and active listening skills.
- Problem-solving skills.
- Time management skills.
- Ability to work in an electronic medical record system.
- Ability to lift 50 lbs.
- Good understanding of body mechanics.
- Strong understanding of professional boundaries.
- Valid driver's license.
- Valid auto insurance.
Nice To Haves
- Current CPR certification.
Responsibilities
- Perform personal care activities related to daily living, including bathing, dressing, grooming, toileting, and skin care.
- Review patient care plans.
- Assist with lifting and transferring patients.
- Assist with range of motion, positioning, exercise, and ambulation.
- Assist with dining and nutrition.
- Maintain electronic medical records.
- Work under the direction of a Case Manager RN.
