About The Position

Collage Home and Community Rehabilitation Partners is seeking experienced Certified Nursing Assistants to provide individualized, patient-centered care in patients’ homes and communities in Houston, Texas, and surrounding areas. The organization provides neurological rehabilitation for people with traumatic brain injuries, spinal cord injuries, CVAs, and other neurological or complex orthopedic diagnoses. Its interdisciplinary teams develop integrated treatment plans and practice functional goals in patients’ homes and communities. PRN positions are available.

Requirements

• Enjoy working one-on-one and be eager to improve and continuously learn new skills.
• Ability to lift and/or move up to 50 pounds and perform job-related physical duties.
• Valid driver’s license and reliable daily transportation.
• Demonstrated ability to act compassionately and supportively.
• Availability to work different shifts, including nights and/or weekends.
• At least one year of prior experience in healthcare, hospitality, or an in-home care environment.
• Willingness to enforce health and safety standards.
• High school diploma or equivalent, experience as a caregiver, CNA, or HHA, or completion of a CNA program.
• Take pride in providing high-quality care.
• Hoyer lift experience, specific to the client.

Responsibilities

• Provide personal hygiene assistance, including help with showering, bathing, dressing, and eating.
• Help clients take prescribed medication and assist with ambulation.
• Help clients move around their homes and outdoors, including to appointments and on walks.
• Assist with personal care and hygiene, plan and prepare meals, and help with physical therapy exercises.
• Complete clients’ shopping or accompany them while shopping.
• Perform housekeeping duties and report unusual incidents.
• Act quickly and responsibly in emergencies.
• Assist patients with meals, including serving food and related tasks as needed.
