About The Position

Collage Home and Community Rehabilitation Partners is seeking experienced Certified Nursing Assistants for PRN work in Southlake, Texas, and surrounding areas. The organization provides neurological rehabilitation for people with traumatic brain injuries, spinal cord injuries, CVAs, and other neurological or complex orthopedic diagnoses in patients’ homes and communities. CNAs work one-on-one with clients as part of interdisciplinary teams and support individualized treatment plans and daily living activities.

Requirements

• At least one year of prior experience in a healthcare, hospitality, or in-home care environment.
• High school diploma or equivalent, experience as a caregiver, CNA, or HHA, or completion of a CNA program.
• Valid driver’s license and reliable daily transportation.
• Ability to lift and/or move up to 50 pounds and perform job-related duties.
• Ability to work different shifts, including nights and/or weekends.
• Willingness to enforce health and safety standards.
• Compassionate and supportive manner, with a commitment to high-quality care.
• Enjoy working one-on-one and be eager to improve and learn new skills.

• Hoyer lift experience, specific to the client.

Responsibilities

• Provide personal hygiene assistance, including help with showering, bathing, dressing, and eating.
• Help clients take prescribed medication and assist with ambulation.
• Support mobility at home and in the community, including accompanying clients to appointments and walks.
• Assist with personal care and hygiene, meal planning and preparation, and physical therapy exercises.
• Complete clients’ shopping or accompany them while shopping.
• Perform housekeeping duties and report unusual incidents.
• Respond quickly and responsibly in emergencies.
• Assist patients with meals, including serving and related tasks as needed.
