About The Position

Moments Hospice is hiring a Certified Nursing Assistant (CNA) to support patient comfort and dignity. The CNA performs personal care and daily-living activities, assists patients with mobility, nutrition, and exercise, and maintains electronic medical records. The role works under the direction of a Case Manager RN and offers a supportive environment with clear expectations, reasonable caseloads, on-call support, and structured training. The position is described as on the road. Compensation is $18–$23 per hour.

Requirements

• Required: Certified Nursing Assistant (CNA) license.
• Required: 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital, or Hospice.
• Excellent customer service skills.
• Strong written and verbal communication and active listening skills.
• Problem-solving skills.
• Time management skills.
• Ability to work in an electronic medical record system.
• Ability to lift 50 lbs.
• Good understanding of body mechanics.
• Strong understanding of professional boundaries.
• Required: Valid driver's license.
• Required: Valid auto insurance.

Nice To Haves

• Current CPR certification (preferred).

Responsibilities

• Perform personal care activities related to daily living, including bathing, dressing, grooming, toileting, and skin care.
• Review patient care plans.
• Assist with lifting and transferring patients.
• Assist with range of motion, positioning, exercise, and ambulation.
• Assist with dining and nutrition.
• Maintain electronic medical records.
• Work under the direction of a Case Manager RN.
