About The Position

Moments Hospice is seeking a Certified Nursing Assistant to support patient comfort and dignity. The CNA performs personal care and daily living activities, assists with mobility, nutrition, and other care-plan needs, and maintains electronic medical records under the direction of a Case Manager RN. The role offers a supportive team environment, reasonable caseloads, on-call support, and structured training. The position is on the road, and a company vehicle benefit is available for business and personal use at minimal cost, with fuel and insurance covered.

Requirements

• Required Certified Nursing Assistant (CNA) license.
• Required 12 months of caregiving experience in home care, assisted living/memory care, skilled nursing/nursing home, hospital, or hospice.
• Excellent customer service skills.
• Strong written, verbal, and active-listening communication skills.
• Problem-solving skills.
• Time management skills.
• Ability to work in an electronic medical record system.
• Ability to lift 50 lbs.
• Good understanding of body mechanics.
• Strong understanding of professional boundaries.
• Required valid driver's license.
• Required valid auto insurance.

Nice To Haves

• Current CPR certification.

Responsibilities

• Perform personal care activities related to daily living, including bathing, dressing, grooming, toileting, and skin care.
• Review patient care plans.
• Assist with lifting and transferring patients.
• Assist with range of motion, positioning, exercise, and ambulation.
• Assist with dining and nutrition.
• Maintain electronic medical records.
• Work under the direction of a Case Manager RN.
