About The Position

Moments Hospice is hiring a full-time, field-based Certified Nursing Assistant (CNA) / Hospice Aide to provide hands-on personal care to hospice patients in private residences, assisted living communities, and care facilities. The role supports patients’ comfort, dignity, and cleanliness while offering companionship to patients and support to their families.

The CNA works closely with a dedicated RN Case Manager as part of an interdisciplinary care team. The position offers structured training and opportunities to build skills. The schedule is Monday through Friday, 8:30 AM to 5:00 PM. Pay is $18–$23 per hour. A fleet car is available for business and personal use, with fuel and insurance covered, at minimal cost and based on eligibility.

Requirements

• Active Certified Nursing Assistant (CNA) certification.
• Ability to work in an electronic medical record system.
• Strong communication skills, including written, verbal, and active listening.
• Excellent customer service, problem-solving, and time management skills.
• Compassion, patience, and the ability to handle emotional situations with care.
• Strong understanding of professional boundaries.
• Ability to lift 50 lbs. and a good understanding of body mechanics.
• Valid driver's license and current auto insurance.

Nice To Haves

• Healthcare caregiving experience in home care, assisted living/memory care, skilled nursing, hospital, or hospice.
• Current CPR certification.

Responsibilities

• Provide personal care with activities of daily living, including bathing, dressing, grooming, toileting, and skin care.
• Assist with lifting, transferring, positioning, range of motion, exercise, and ambulation.
• Assist with dining and nutrition.
• Review and follow each patient's plan of care under the direction of the RN Case Manager.
• Provide comfort and companionship to patients and support to their families.
• Maintain electronic medical records.
