About The Position

Moments Hospice is seeking a Certified Nursing Assistant (CNA), also described as a Hospice Aide, to provide hands-on personal care to hospice patients in their homes, assisted living communities, or care facilities. The role focuses on patients’ comfort, cleanliness, dignity, and companionship, while also supporting their families.

The CNA works closely with a dedicated RN Case Manager as part of an interdisciplinary care team. The position is field-based and offers structured training and opportunities to build skills. The schedule is Monday through Friday, 8:30 AM to 5:00 PM. Compensation is $18–$23 per hour. A fleet car is provided for business and personal use, with fuel and insurance covered, at minimal cost to the employee.

Requirements

• Active Certified Nursing Assistant (CNA) certification.
• Ability to work in an electronic medical record system.
• Strong written and verbal communication and active listening skills.
• Excellent customer service, problem-solving, and time management skills.
• Compassion, patience, and ability to handle emotional situations with care.
• Strong understanding of professional boundaries.
• Ability to lift 50 lbs. and a good understanding of body mechanics.
• Valid driver's license and current auto insurance.

Nice To Haves

• Healthcare caregiving experience in home care, assisted living/memory care, skilled nursing, hospital, or hospice.
• Current CPR certification.

Responsibilities

• Provide personal care with activities of daily living, including bathing, dressing, grooming, toileting, and skin care.
• Assist with lifting, transferring, positioning, range of motion, exercise, and ambulation.
• Assist with dining and nutrition.
• Review and follow each patient's plan of care under the direction of the RN Case Manager.
• Provide comfort and companionship to patients and support to their families.
• Maintain electronic medical records.
