About The Position

Moments Hospice is hiring a full-time, field-based Certified Nursing Assistant (CNA) / Hospice Aide to provide hands-on personal care to hospice patients in private residences, assisted living settings, and care facilities. The role supports patients’ comfort and dignity and provides reassurance to their families. The CNA works closely with an RN Case Manager as part of an interdisciplinary care team, following each patient’s plan of care under the RN’s direction. The position includes structured training and is described as having clear expectations and reasonable caseloads.

The schedule is Monday through Friday, 8:30 AM to 5:00 PM. Compensation is $18–$23 per hour. A fleet car program provides a company vehicle for business and personal use, with fuel and insurance covered at minimal cost to the employee.

Requirements

• Active Certified Nursing Assistant (CNA) certification is required.
• Valid driver’s license and current auto insurance are required.
• Ability to work in an electronic medical record system.
• Strong written and verbal communication skills, including active listening.
• Excellent customer service, problem-solving, and time management skills.
• Compassion, patience, and the ability to handle emotional situations with care.
• Strong understanding of professional boundaries.
• Ability to lift 50 lbs. and a good understanding of body mechanics.

Nice To Haves

• Healthcare caregiving experience in home care, assisted living/memory care, skilled nursing, a hospital, or hospice is a plus.
• Current CPR certification is preferred.

Responsibilities

• Provide personal care with activities of daily living, including bathing, dressing, grooming, toileting, and skin care.
• Assist with lifting, transferring, positioning, range of motion, exercise, and ambulation.
• Assist with dining and nutrition.
• Review and follow each patient’s plan of care under the direction of the RN Case Manager.
• Provide comfort and companionship to patients and support to their families.
• Maintain electronic medical records.
