About The Position

San Francisco Post-Acute is seeking compassionate, dependable, and team-oriented Certified Nursing Assistants to provide exceptional, resident-centered care in a fast-paced post-acute and skilled nursing environment. The role involves working collaboratively with a dedicated healthcare team and supporting residents’ comfort, dignity, and independence. Part-time shifts are available: AM shift on weekdays, PM shift on weekends, and NOC shift on weekdays and weekends. The posted pay rate is $24.00–$26.00 per hour, depending on experience. Requirements

Current and active Certified Nursing Assistant (CNA) certification.
Strong communication and interpersonal skills.
Ability to work effectively in a team-oriented healthcare environment.

Nice To Haves

Previous skilled nursing, rehabilitation, or post-acute experience is preferred but not required.

Responsibilities

Monitor resident conditions and promptly report changes to the nursing team.
Respond quickly and professionally to resident call lights and requests.
Assist residents with activities of daily living while promoting comfort, dignity, and independence.
Support admissions, transfers, and discharge procedures.
Assist with meal service and tray distribution as needed.
Maintain accurate documentation and follow facility policies and procedures.
Collaborate with nurses, therapists, physicians, and other team members to provide resident care.
Help maintain a safe, clean, and welcoming environment for residents.
