Certified Nursing Assistant - Weekends
About The Position
Sangre de Cristo Community Care is seeking a compassionate and dependable Certified Nursing Assistant (CNA) for its hospice home care team in Pueblo, Colorado. The role provides meaningful, one-on-one care to patients in their homes as part of an interdisciplinary hospice team focused on comfort, dignity, and quality of life. The CNA works under the direction of an RN or Case Manager and follows each patient's individualized Plan of Care. The position is scheduled Friday, Saturday, and Sunday during the day, including holidays that fall within the scheduled rotation. The CNA travels to patients' homes and other care settings throughout the assigned service area.
Requirements
- Current Colorado CNA certification in good standing.
- Valid driver's license and reliable transportation.
- Compassionate and dependable, and comfortable providing end-of-life care.
- Strong communication and time-management skills.
- Ability to work independently while remaining connected to the clinical team.
- Availability to work the Friday–Sunday day schedule, including holidays that fall within the scheduled rotation.
Nice To Haves
- At least one year of CNA experience.
- Hospice, home health, or other direct patient care experience.
Responsibilities
- Provide personal care, including bathing, grooming, dressing, and toileting.
- Assist with mobility, transfers, positioning, and ambulation.
- Assist with meals, hydration, and nutritional needs.
- Observe patients and promptly report changes in condition to the nursing team.
- Help maintain a safe, clean, and comfortable environment for patients.
- Travel to patients' homes and other care settings throughout the assigned service area.
- Provide compassionate support while respecting each patient's dignity, privacy, and personal preferences.
- Communicate and collaborate with nurses and other members of the interdisciplinary hospice team.
- Build trusting relationships with patients and families.
