About The Position
Ascend Health is seeking a compassionate and dedicated Certified Nursing Assistant (CNA) to join its Home Health and Hospice team. The CNA provides hands-on care and support to patients and families in their homes, delivering care with dignity and compassion. The role serves the Pocatello and Idaho Falls area and is part-time.
Requirements
- Current CNA certification in Idaho, or ability to obtain it.
- Compassionate, patient, and reliable, with strong communication skills.
- Valid driver's license, reliable vehicle, and current auto insurance.
- Ability to work independently while being part of a supportive team.
Nice To Haves
- Previous experience in home health, hospice, or patient care.
Responsibilities
- Provide personal care, companionship, and support to patients in their homes.
- Assist patients with activities of daily living, including bathing, dressing, grooming, feeding, mobility, and toileting.
- Monitor and report changes in patient condition to the supervising nurse.
- Provide emotional support to patients and families.
- Document patient care and maintain accurate, timely records in the EMR.
- Follow the plan of care as directed by the clinical team.
