About The Position

Bluegrass Care and Rehab Center is a 124-bed facility providing skilled nursing and long-term care. The Certified Nursing Assistant provides direct resident care under the supervision of licensed nursing personnel and helps promote a compassionate physical and psychosocial environment in accordance with facility policies and procedures. The role supports residents’ activities of daily living within the scope of practice and their plans of care, while respecting resident rights and choices. The position is part of Signature HealthCARE, a family-based healthcare company offering integrated care services across multiple states.

Requirements

• Current, active Nursing Assistant certification from the state in which the facility operates.
• Effective verbal and written communication skills.
• Ability to work collaboratively within a nursing team and under the direction of licensed nursing staff.
• Customer-service orientation and ability to work well under pressure.
• Approachable, flexible, and adaptable to change.
• Ability to function independently, demonstrate personal integrity, and work effectively with stakeholders and vendors.

Responsibilities

• Perform direct resident care duties under the supervision of licensed nursing personnel.
• Help promote a compassionate physical and psychosocial environment in accordance with facility policies and procedures.
• Provide activities of daily living within the scope of practice and the resident’s plan of care.
• Change bed linen and maintain appropriate grooming supplies in resident rooms.
• Ambulate and transfer residents using appropriate assistive devices and body mechanics.
• Record resident vital signs, weights and measurements, intake and output, food consumption, and other assigned information.
• Routinely turn and position residents as necessary.
• Recognize and support resident rights and choices within the care environment.
• Maintain established housekeeping standards within assigned duty areas.
• Promptly answer call lights and assist with resident needs.
• Assist in orienting and training other staff as requested.
• Perform other special projects and duties as assigned.
