About The Position

Moments Hospice is hiring a Certified Nursing Assistant (CNA) to support patient comfort and dignity. The CNA performs personal care and daily living activities, reviews care plans, assists with patient mobility and nutrition, and maintains electronic medical records under the direction of a Case Manager RN. The role offers a supportive team environment, reasonable caseloads, on-call support, and structured training. Requirements

• Required Certified Nursing Assistant (CNA) license.
• Required 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital, or Hospice.
• Excellent customer service skills.
• Strong written and verbal communication and active listening skills.
• Problem-solving skills.
• Time management skills.
• Ability to work in an electronic medical record system.
• Ability to lift 50 lbs.
• Good understanding of body mechanics.
• Strong understanding of professional boundaries.
• Valid driver's license required.
• Valid auto insurance required.

Nice To Haves

• Current CPR certification is preferred.

Responsibilities

• Perform personal care activities related to daily living, including bathing, dressing, grooming, toileting, and skin care.
• Review patient care plans.
• Assist with lifting and transferring patients.
• Assist with range of motion, positioning, exercise, and ambulation.
• Assist with dining and nutrition.
• Maintain electronic medical records.
• Work under the direction of a Case Manager RN.
