About The Position

Elara Caring is seeking a compassionate Certified Nurse Aide to provide home care. The role provides hands-on personal care under the direction of a Registered Nurse, supporting patients with daily activities, mobility, safety, and adherence to individualized care plans. The aide also monitors and reports changes in patient condition, helps maintain a safe and comfortable home environment, and documents care while protecting patient privacy.

Requirements

• Current state certificate as a Certified Nursing Aide.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license and maintain an acceptable driving record, or have reliable transportation to travel between patient homes and assigned work locations.

Responsibilities

• Provide hands-on personal care under the direction of a Registered Nurse.
• Assist patients with bathing, dressing, grooming, toileting, and other daily activities.
• Take and record vital signs, including temperature, pulse, respirations, and blood pressure.
• Help patients safely walk, transfer, and use mobility equipment.
• Follow each patient's individualized care plan.
• Monitor and report changes in a patient's condition to the care team.
• Help maintain a clean, safe, and comfortable home environment.
• Remind patients to take medications as directed.
• Respond appropriately to emergency situations and follow safety and infection control guidelines.
• Maintain accurate documentation, protect patient privacy, and participate in required training.
