About The Position

Moments Hospice is seeking a Certified Nursing Assistant (CNA), also referred to as a Hospice Aide, to provide hands-on personal care to hospice patients in their homes, assisted living settings, and care facilities. The role supports patients’ comfort, dignity, and well-being and provides support to their families.

The CNA works closely with a dedicated RN Case Manager as part of an interdisciplinary care team. The position is field-based and offers structured training and opportunities to build skills. The schedule is Monday through Friday, 8:30 AM to 5:00 PM. A fleet vehicle is available for business and personal use, with fuel and insurance covered at minimal cost to the employee.

Requirements

• Active Certified Nursing Assistant (CNA) certification is required.
• Ability to work in an electronic medical record system.
• Strong written and verbal communication skills, including active listening.
• Excellent customer service, problem-solving, and time management skills.
• Compassion, patience, and the ability to handle emotional situations with care.
• Strong understanding of professional boundaries.
• Ability to lift 50 lbs. and a good understanding of body mechanics.
• Valid driver's license and current auto insurance are required.

Nice To Haves

• Healthcare caregiving experience in home care, assisted living or memory care, skilled nursing, hospital, or hospice.
• Current CPR certification.

Responsibilities

• Provide personal care with activities of daily living, including bathing, dressing, grooming, toileting, and skin care.
• Assist with lifting, transferring, positioning, range of motion, exercise, and ambulation.
• Assist with dining and nutrition.
• Review and follow each patient's plan of care under the direction of the RN Case Manager.
• Provide comfort and companionship to patients and support to their families.
• Maintain electronic medical records.
