About The Position

The Hospice Certified Nursing Assistant provides paraprofessional services to the organization’s home care clients, as developed by the primary nurse and prescribed by the physician. Services must comply with the state’s Nurse Practice Act and other regulations governing home health aide and hospice aide services. The position serves Upper Montgomery County and provides meaningful one-to-one patient care in clients’ homes.

Requirements

High school diploma or GED.
Completion of a federally compliant training program as required by law or regulation, and successful completion of a competency evaluation program.
Documentation of one year of employment as an aide in an institution, home care, or hospice setting.
CPR certification.
Ability to function with minimal direct supervision and accept personal responsibility for maintaining professional relationships with clients and their families.
Current driver’s license and reliable transportation to and from the work site.
Meet agency hiring criteria, including provision of professional references.

Responsibilities

Provide paraprofessional services to home care clients as developed by the primary nurse and prescribed by the physician.
Provide services in compliance with the state’s Nurse Practice Act and regulations governing home health aide and hospice aide services.
Maintain a professional relationship with agency clients and their families while functioning with minimal direct supervision.
Maintain skills, continue learning, and follow agency policies and procedures on an ongoing basis.
Demonstrate organizational core competencies and code of ethics, and adhere to the compliance program.
