About The Position

Moments Hospice is seeking a Certified Nursing Assistant to support patient comfort and dignity. The CNA performs personal care and other activities of daily living, reviews care plans, and assists with patient mobility, exercise, nutrition, and related needs. The position works under the direction of a Case Manager RN and maintains electronic medical records. The role is on the road and includes a fleet car benefit option with gas and insurance covered; the vehicle may be used for business and personal purposes at a minimal cost. A company cell phone, clear expectations, reasonable caseloads, on-call support, and structured training are also described.

Requirements

• Certified Nursing Assistant (CNA) license.
• 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital, or Hospice.
• Excellent customer service skills.
• Strong written, verbal, and active-listening communication skills.
• Problem-solving skills.
• Time management skills.
• Ability to work in an electronic medical record system.
• Ability to lift 50 lbs.
• Good understanding of body mechanics.
• Strong understanding of professional boundaries.
• Valid driver's license.
• Valid auto insurance.

Nice To Haves

• Current CPR certification.

Responsibilities

• Perform personal care activities related to daily living, including bathing, dressing, grooming, toileting, and skin care.
• Review patient care plans.
• Assist with lifting and transferring patients.
• Assist with range of motion, positioning, exercise, and ambulation.
• Assist with dining and nutrition.
• Maintain electronic medical records.
• Work under the direction of a Case Manager RN.
