About The Position

This position is an advanced Community Health Worker responsible for working largely independently communicating with Care Transitions staff members, University Hospitals Patients, Providers and Community Health Worker Partnered Sites. Working under minimal supervision in a professional environment, the role involves providing patient education and connecting patients with available resources within Care Transitions, University Hospitals, and the community to assist patients in identifying and overcoming barriers.

Requirements

  • High School Equivalent / GED
  • 1+ years as a Community Health Worker in a clinical and or community setting
  • 1+ years progressive work experience in a healthcare environment
  • Experience working multiple healthcare data sources that include Electronic Medical Record application(s), registration and scheduling systems, and medical claims systems
  • Experience facilitating telephonic patient outreach
  • Competent in all practice areas
  • Basic medical terminology
  • High integrity level and ability to maintain confidentiality of sensitive data
  • Excellent verbal and written communication skills
  • Proven time management and organizational skills
  • Demonstrated excellence in customer service with all stakeholders (Patient, employer staff, providers, etc.)
  • Demonstrated ability to apply concepts, utilize sound judgment and work independently
  • Ability to work within a team environment and maintain productivity and professionalism working remotely under minimal day-to-day supervision
  • Knowledge of healthcare resources, benefits and entitlements in the local area
  • Ability to handle multiple initiatives and priorities within a fast-paced environment
  • Ability to keep composure and professionalism during times of high emotional stress
  • Good computer skills with the ability to utilize Microsoft Office products
  • Community Health Worker (CHW) Completion of state education and certification has been issued by the board and verified by supervisor

Responsibilities

  • Provide patient education, connect patients with available resources within Care Transitions, UH, and in the community to assist patients in identifying and overcoming barriers
  • Coordinate with patient to assess and identify community resources needed and assures appropriateness of referral source for patient
  • Actively take on complex care cases within CHW scope of practice
  • Actively interact with Social Worker, RN, care team and Provider to facilitate patient care coordination
  • Be active in the community in volunteer capacity and utilize community connections to facilitate patient care
  • Serve as a liaison between patient and patient’s family when appropriate and patient internal/external care team
  • Address missed, declined, and rescheduled visits with patient to assess for barriers to attendance of appointments
  • Understand PHI and HIPAA and actively protect patient confidentiality and uses appropriate points of contacts and consenting
  • Initiate and maintain ongoing communication between disciplines and referral sources connected to
  • Communicate with Patients as they matriculate throughout the healthcare system and community
  • Demonstrate ability to collaborate among peers and system resources
  • Directly and promptly address patient concerns for internal/external resource supports
  • Advocate for patients needs professionally and is capable of representing the patient as an advocate
  • Be able to identify patient safety concerns and collaborate with Social Worker to address high level concerns
  • Refer only complex and/or clinical issues to Social Worker for follow up
  • Demonstrate knowledge of patient history and provides high level over of patient history based on chart research
  • Manage daily, weekly assigned tasks, documentation, follow up calls and reports
  • Maintain professionalism and community health worker standard of professionalism and ethics in high stress situations and complex cases
  • Demonstrate interpersonal and communication skills, and emotional intelligence when working with patients/caregivers and clinicians at all times
  • Possess proficiency in information technology and appropriate databases to facilitate timely completion of referrals, timely documentation and ensure resource connection
  • Perform other duties as assigned
  • Comply with all policies and standards
  • Refer to documentation provided by the department during orientation for specific duties and responsibilities
  • Abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients
