About The Position

Under the supervision of the Family Preservation Clinical Coordinator, the Case Manager provides comprehensive family assessments and delivers in-home, strengths-based services to families enrolled in the Family Preservation Program. This role partners closely with families and a multidisciplinary team to develop and implement individualized service plans that promote child safety, family stability, and overall well-being. All services are delivered in alignment with best practices and contractual requirements set forth by the Department of Children and Family Services (DCFS).

Requirements

  • Bachelor's degree in social work, psychology, sociology, counseling or related field from an accredited college or university is required.
  • One year of case management experience is required, but education may be considered in lieu of formal experience.
  • Knowledge of child abuse/crisis intervention techniques
  • Strong written and oral communication skills in English and Spanish are required.
  • Utilizes Microsoft Office Suite and other business systems to perform daily administrative functions, including data entry, document management, and professional correspondence.
  • Travel Requirements: Up to 75% within Los Angeles County requiring a valid driver's license, personal vehicle, and automobile insurance.
  • Must be able to speak and hear well.
  • Good vision is imperative.
  • Employee will be required to lift and/or move up to 25 pounds without assistance.
  • TB test clearance to be renewed every two years.
  • Verification of Immunizations required by the California Department of Health (Pertussis, Measles and Influenza).

Responsibilities

  • Conducts in-home visits to assess family needs, build rapport, and support clients in achieving established case plan goals.
  • Collaborates with multidisciplinary team members and clinical supervisors to develop, implement, and monitor individualized service and treatment plans.
  • Co-facilitates case conferences to review client progress, address barriers, and adjust interventions as needed.
  • Connects families to community-based resources and supports that enhance family functioning and strengthen parenting capacity.
  • Establishes and maintains respectful, culturally responsive relationships with children and families from diverse backgrounds.
  • Maintains accurate, up-to-date client records in compliance with agency policies and regulatory standards.
  • Completes all required documentation in a timely manner, including Family Assessment Forms (FAFs), weekly progress notes, monthly reports, and discharge summaries.
  • Ensures strict adherence to confidentiality standards in accordance with state laws and agency guidelines.
  • Participates in required staff meetings, trainings, supervision sessions, and professional development opportunities.
  • Engages in weekly individual and group supervision to support professional growth and service quality.
  • Fosters strong, collaborative relationships with internal staff and external partners.
  • Contributes to Quality Assurance (QA) and Continuous Quality Improvement (CQI) efforts to enhance program effectiveness and outcomes.
