About The Position

Merakey is seeking a Case Manager to join their Children's Behavioral Health Program serving Clients in Kent County, Delaware. The Case Manager will coordinate resources and information pertinent to client needs, document coordination of resources, and manage referrals for client-specific discharge planning, ensuring family participation. They will also monitor record keeping, compliance, and authorized units, ensuring client charts are up-to-date with necessary documentation, including incident reports within 24 hours. The role involves monitoring and assessing document safety and completing necessary case documentation in compliance with Merakey, county MHDS/CASSP, OMHSAS, and other licensing/regulatory/payor organizations' standards. Case sessions are primarily delivered in the community (client homes, schools, etc.), but can also be in an office environment. The position may require working evenings and/or weekends to meet client needs and may include on-call coverage.

Requirements

• Bachelor's degree
• At least 1 year of experience working with children/adolescents
• Valid Driver's License and clean driving record
• Able to work Monday-Friday with an ON-CALL rotation

Responsibilities

• Coordinate resources and information pertinent to client needs.
• Effectively and efficiently documents all coordination of resources.
• Coordinate referrals for client specific discharge planning; ensuring that the family actively participates in creating a multi-faceted discharge plan; one which represents the family’s choice for service providers, and community resources.
• Monitor record keeping, compliance and authorized units remaining, ensures that client charts are up-to-date and include all necessary documentation, including documentation of significant incidents within 24 hours as needed (incident reports).
• Monitor/assess documents safety
• Complete necessary documentation of each case, which complies with the standards set by Merakey, county MHDS/CASSP, OMHSAS, and other licensing/regulatory/payor organizations.
