About The Position

The Case Manager will be responsible for overseeing guests enrolled in the PRP program, providing Christ-centered counsel, monitoring their progress, and ensuring they meet program requirements. This role involves collaborating with guests, the Case Management team, and external agencies to facilitate guest success and determine length of stay. The Case Manager will also teach PRP classes, maintain accurate records, assist guests with money management, and report maintenance concerns. Additionally, the position requires being on call for guest emergencies and coordinating drug and alcohol testing.

  • Ability to maintain confidentiality of all Crossroads activities, guests, donors, and staff information.
  • Ability to maintain composure in stressful situations.
  • Proven computer skills and willingness to become proficient with Crossroads Mission Avenue computer software.
  • High School diploma or equivalent.
  • Hold a current CPR and First Aid Certification, or willingness to become certified.

Responsibilities

  • Meet with guests enrolled in the PRP program a minimum of one time per week to monitor and encourage their progress.
  • Provide Christ-centered counsel that supports guest recovery.
  • Confirm guest fulfillment of each program phase requirement.
  • Refer guests to and assist them in contacting area agencies and organizations with which Crossroads partners.
  • Engage in teamwork with guest, Operations Manager, and Case Management team to determine each assigned guest’s length of stay according to their efforts to succeed in their program and their current situation.
  • Develop and maintain professional relationships with local agencies and organizations, and be familiar with their services to make informed referrals.
  • Teach PRP classes on a rotational and assigned basis.
  • Communicate pertinent information concerning each guest’s welfare, progress, and status of stay to Operations Manager, Lead Case Manager, and partnering agencies.
  • Lead current guests from a Christian perspective, empowering them to affect needed change in their lives and encouraging them to grow in their relationship with Christ.
  • Identify gaps in services and communicate observations to the supervisor.
  • Work with Lead Case Manager and Executive Staff to develop goals for Crossroads’ 4 Phase Transitional Living Program.
  • Be on call on a rotational basis in case of emergencies involving guests.
  • Work with Operations Manager to coordinate guest drug and alcohol testing as needed.
  • Log and file program records neatly and properly, keeping guest information secure at all times.
  • Assist guests with money management.
  • Report maintenance concerns to Operations Manager using Crossroads Mission Avenue maintenance concerns protocol.
  • Perform other duties as assigned.
