About The Position

The Case Manager will develop and coordinate resident programming to increase independence, self-reliance, and self-determination and will monitor and evaluate ongoing programming for residents to ensure efficacy and relevance. The Case Manager will assist residents and the resident community in the development of interpersonal, social, life, and other skills through the facilitation of workshops, groups, and presentations including resident meetings. The Case Manager will provide direct, one to one support for a small caseload of residents through client-centred intakes and assessments; case planning; referrals to other resources and services as necessary; developmentally appropriate life skills training in areas related to housing, independent living, financial literacy/budgeting, health and nutrition, family and community relationships, sexual health, medical and mental health; counselling; substance use support; support for employment, and education.

Requirements

  • Post-secondary degree/diploma in social work, social service work, child and youth work, or related discipline or equivalent experience.
  • 2+ years of professional experience, preferably working with marginalized youth.
  • Ability to critically analyze personal and systemic issues leading youth to become homeless.
  • Ability to integrate anti-oppression, critical disability, 2SLGBTQ+ and trans-affirming, trauma, harm reduction, and strengths-based principles into practice.
  • Experience in resident program planning, development, implementation, and facilitation.
  • Strong organizational and administrative skills.
  • Knowledge of transitional and other housing systems in Toronto; ability to assist youth transitioning into adulthood through developmentally appropriate support.
  • Relevant knowledge of community resources and referral agencies.
  • Proven ability to coordinate case management services resulting in seamless service delivery for residents.
  • Ability to advocate effectively with other systems resulting in increased access for residents.
  • Well-developed interpersonal and relationship-building skills; ability to establish rapport and excellent communication with members, staff, and volunteers.
  • Excellent written and oral communication skills.
  • Flexibility to work hours, including evenings and weekends as required.
  • Excellent understanding of diversity, inclusion and equity including the ability to identify and address barriers, work effectively with diverse stakeholders, and integrate equity into program design

Responsibilities

  • Conduct intakes, interviews, and assess integrating strengths-based frameworks.
  • Collaboratively identify and co-create short and long-term resident goals.
  • Coordinate case management and support planning with residents, the team, formal and informal supports and systems.
  • Ensure ongoing evaluation and effectiveness of case management, service, and program outcomes for residents.

  • Adhere to all protocols and practices at your location as well as Association and other relevant policies and procedures.
