As a Part-Time Medical Social Worker, you’ll be a vital member of the care coordination team, collaborating closely with RN case managers, multidisciplinary clinical teams, and internal and external partners. You will perform comprehensive psychosocial assessments, provide supportive counseling, coordinate care related to discharge planning and service coordination, and support adult, geriatric, pediatric, and med/surg populations in a fast-paced, patient-centered hospital setting.

Requirements

• A Master’s degree in Social Work (MSW)
• Current Massachusetts Social Work licensure (LCSW)
• A minimum of one year related experience in a hospital environment and/or with transferable skills that apply to a fast-paced and high-urgency setting
• Ability to triage crises and collaborate/coordinate with collateral agencies

Nice To Haves

• Experience with protective services, child welfare, long-term care, crisis intervention, chronic illness, hospice care, substance use disorders, and psychiatric illnesses is a plus
