About The Position

We are seeking a Mental Health Worker to join our team at our program in Monaca, PA, supporting Beaver County. Earn $20.75 an hour + mileage reimbursement. Assertive Community Treatment (ACT) Teams are composed of professionals, including psychiatrists, nurses, mental health professionals, substance abuse professionals, employment specialists, housing specialists, and a peer specialist, all of whom collaborate to provide complete services, including therapy, medication management, and resource linkage to adult individuals with serious mental illness (SMI). Services are provided in the individual's home or the community and include at least three home visits a week. Will be required to utilize personal vehicles for client transportation and community-based work.

Requirements

• Bachelor's Degree in a human service field
• 1 year experience working with individuals in a recovery setting and/or crisis experience.
• Valid PA Driver's License.
• Travel is necessary to offer support across Beaver County.

Responsibilities

• Provide rehabilitation, support, and direct care services to individuals with mental health and behavioral health needs, including assistance with daily living activities and therapeutic interventions.
• Implement treatment plans under clinical supervision, including substance use services, education, family consultation, and crisis intervention support as needed.
• Collaborate with families, healthcare providers, social service agencies, and community partners to coordinate comprehensive care and support positive outcomes.
• Ensure the safety and well-being of individuals served by following established policies, procedures, and safety protocols.
• Participate in treatment planning and work collaboratively with the interdisciplinary team to support recovery and treatment goals.
