About The Position

Metropolitan Family Services (MFS) is seeking a full-time Case Manager for its South Chicago office. MFS is an organization that empowers families to learn, earn, heal, and thrive, with a mission dating back to 1857. The role focuses on making a positive impact on families and communities.

Requirements

  • Bachelor's Degree in Psychology, Social Work, or related field from an accredited college or university, required.
  • Ability to work evening and weekend hours as needed.
  • Satisfactorily pass all relevant screening and criminal background checks.
  • Driving for work required.

Nice To Haves

  • One year of case management experience in a social service or healthcare setting preferred.
  • Certified Alcohol and Drug Counselor certification preferred.
  • Bilingual in Spanish preferred.
  • Crisis stabilization experience preferred.
  • Transportation of clients preferred in a personally owned vehicle.

Responsibilities

  • Conducts thorough assessment of clients.
  • Evaluates problem and service needs.
  • Develops a written client service plan specifying goals to be achieved and services to be pursued.
  • Supplements service plans to meet client needs.
  • Provides supportive help, advocacy, and case management to individuals and families in coping with environment and interpersonal relationship issues.
  • Works collaboratively with community resources and develops a knowledge of resources to help clients utilize them.
  • Works within contract, grant, and program requirements, including meeting timelines and reporting requirements.
  • Engages clients in challenging settings to learn/practice healthy behaviors and coping skills.
  • Reassesses and monitors client situations and the adequacy of service plans.
  • Collaborates and participates with other program staff in planning and implementing service plans.
  • Handles emergency and crisis situations within the target population after consultation with a supervisor.
  • Authorizes limited amounts of financial assistance to own clients with supervisory approval.
  • May work with community groups.
  • Works effectively with diverse staff and service population.
  • Other duties as assigned.
