As a Case Manager supporting our customer, you will be trusted to provide clinical support. The Case Manager provides comprehensive coordination of healthcare and support services for personnel and their families. This role ensures continuity of care by integrating medical and non‑medical resources, tracking progress, supporting multidisciplinary teams, and facilitating effective transitions between levels of care. The Case Manager conducts assessments, develops individualized plans, coordinates services, and evaluates outcomes to promote optimal health, function, and readiness.
