Case Manager - FT | Aiken Rehab
About The Position
PAM Health is hiring a full-time Case Manager for its new rehabilitation hospital in Aiken, South Carolina. The Case Manager coordinates care across an interdisciplinary team for a designated patient population. The role reviews inpatient admission records to assess resource utilization and whether patients are admitted at the appropriate level of care, identifies discharge needs, and coordinates appropriate discharge plans. The Case Manager collaborates with the interdisciplinary team to support desired financial and quality outcomes and maintains and enhances payer relationships. The position is open to professionals in designated disciplines including occupational therapy, physical therapy, nursing, respiratory therapy, speech therapy, and licensed social work. State licensure is preferred, and current BLS certification is required. Three to five years of inpatient experience is required, preferably in an acute, IRF, or LTACH setting.
Requirements
- Current BLS certification.
- Three to five years of inpatient experience.
- Professional discipline may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or licensed social worker.
Nice To Haves
- State licensure in the state where the hospital resides is preferred.
- Inpatient experience in an acute, IRF, or LTACH setting is preferred.
