This position will provide diverse case management to veterans experiencing disability, justice involvement, poverty, and/or homelessness enrolled in our employment programs. Services for participants include coordination of vocational and pre-employment individual and group services along with Employment Development Specialist. A widely varied resource/referral knowledge to support participants identify job goals, execute effective job searches, obtain suitable and appropriate employment, and retain employment over time. This position will also facilitate quarterly needs assessment to alleviate barriers to employment.

Requirements

  • High school diploma or general education degree (GED) and 2 years’ experience in case management required.
  • Considerable experience in crisis intervention, trauma informed care practices, supporting individuals experiencing poverty, homelessness and disabilities as they strive to obtain and retain employment, money management, benefits assistance issues, an understanding of mental illness, and how these influence employment situations.
  • Excellent Microsoft Office skills, especially Excel, Outlook, and Word
  • Ability to work in multiple databases, and proficient in data entry
  • Excellent communication skills (written and verbal), grammar, and reasoning skills
  • Understanding of the socioeconomic conditions that impact conditions for people experiencing homelessness, justice involvement, disability, and severe poverty
  • Ability to handle confidential and sensitive information without breach
  • Personal commitment to working within an integrated services model, with emphasis on employment as a key component to ending poverty and homelessness.
  • Ability to cultivate strong working relationships with community resources.
  • Knowledge of applying for all public benefits available in the state of Colorado.
  • Knowledge of community resources available to participants such as food banks, healthcare options, linkage to housing, etc.
  • Ability to connect participants to any other services they may need in the community.

Responsibilities

  • Serve as Case Manager within the employment team, working with participants to provide an array of services and referrals to best meet participant needs.
  • Provide support and advocacy services for participants to maintain access to public benefits and/or community resources.
  • Educate individuals on how income may affect other human service benefits they may receive.
  • Assist participants navigate any barriers that arise: a missed court date, an active warrant, loss of personal belongings, etc. and support them through the process of addressing these hurdles.
  • Complete all documentation and data entry as required in a timely manner.
  • Act as the team’s intake professional, being the first point of contact; creating files in databases, conducting initial assessments, and setting out the case management plan.
  • Updating the file as milestones are met, or as new needs arise.
  • Work collaboratively with a wide variety of constituents and adjusts to multiple demands and shifting priorities.
  • Develop a good rapport with participants and understand hurdles caused by or exacerbated by disability, justice involvement, poverty, and/or homelessness.
  • Utilizing trauma informed practices, connecting participants to resources relevant to the individual.
  • Assist participants in developing money management and budgeting skills.
  • Assist participants in developing technological literacy skills.
  • Assist participants navigate concrete and social issues that may impede their ability to work.
  • Attend applicable staff and community meetings.
  • Performs other duties to include jobsite coverage as assigned by management.
