About The Position

The Case Manager is responsible for interdisciplinary coordination of care for a designated patient population. The role includes reviewing inpatient admission records to ensure appropriate use of hospital resources and determine whether admissions are at the appropriate level of care. The Case Manager assesses discharge needs and coordinates appropriate discharge plans, working collaboratively with the interdisciplinary team to achieve desired financial and quality outcomes. The position also maintains and enhances payor relationships.

Requirements

Current BLS certification is required.
Three to five years of inpatient experience.

Nice To Haves

Licensure in the state where the hospital is located is preferred.
Inpatient experience in an acute, IRF, or LTACH setting is preferred.
Professional background as an occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or licensed social worker.
