About The Position

This position has primary responsibility for case management services to clients and their families referred through Stars’ Transition Age Youth (TAY) Program (TAY). The Transition Age Youth (TAY) Program is an outpatient, community-based program that serves youth age 18-25 with serious and persistent mental health issues. The program provides intensive case management, outreach and engagement, medication support, as well as mental health counseling, peer groups, and supportive services to assist TAY transition to adult life. Services offered are trauma informed and culturally responsive.

Requirements

4 years of experience in a social service field OR Associate degree plus 2 years of experience in the social service field OR Bachelor’s degree in a social science field plus at least 6 months experience in a social service field or a school internship in a social service field
Valid California Driver’s License
Current automobile insurance
Must maintain a driving record that meets the company driver's eligibility policy.

Supervision for BBS hours for AMFT, ACSW, and APCC professionals (where applicable).
