About The Position

New River Valley Community Services Medical Services is seeking a Case Manager to work with the MAT (Medication Assisted Treatment) Team. The Case Manager provides needs assessments, connects individuals with resources, and monitors services for people with substance use and/or co-occurring disorders. The role coordinates services and resources to help clients remain in their communities and monitors service delivery to ensure consumers’ needs are met. The Case Manager also ensures documentation requirements are met and assists with transportation and community integration as needed.

Requirements

• Bachelor’s degree in education, social work, psychology, or a related human services field.
• QMHP certification through the Virginia Board of Counseling is required.

Nice To Haves

• At least one year of experience working with individuals with disabilities, mental health needs, substance use, or Developmental Disabilities is preferred.
• CSAC is beneficial.
• Previous Case Management experience is preferred.
• Additional compensation may be considered for new employees who speak multiple languages.
