About The Position

The Case Manager provides social work and client advocacy services to clients of Big Bend Cares. This position is responsible to the Director of Client Services. The Case Manager assists HIV positive clients with improving their health by providing client-centered activities, including comprehensive assessment of service needs, development of a comprehensive, individualized care plan, and providing timely and coordinated linkage and access to core medical and support services. The role involves monitoring client progress, re-evaluating care plans, and adhering to DOH’s HIV/AIDS case management standards and guidelines. The Case Manager must maintain a thorough knowledge of client rights and responsibilities, advocate for clients in accessing needed services, and recommend changes in case management policies. Responsibilities also include completing pre-intakes, maintaining and reporting caseload data, managing client files, and completing billing documentation.

Requirements

  • A bachelor’s degree in psychology, social work, or human services, OR a bachelor’s degree in any field of study with at least six months of case management experience, OR applicable experience (year-for-year substitution for education), OR a registered nursing license in Florida.
  • Must submit to and pass a level 2 background screening.
  • Must have reliable transportation.
