About The Position
Valley’s Forensics program empowers individuals to build stability, strengthen independence, and move toward meaningful life goals. As a Case Manager I, you'll partner with clients to connect them with resources, navigate challenges, and develop the skills needed for long-term success. Through personalized support, advocacy, and evidence-based case management, you'll help clients make lasting progress on their path to recovery and well-being.
Requirements
- Case Manager certification (post hire as needed - required within 120 days of start date)
- CPR certification (post hire as needed)
- Driving positions require a minimum age of 21 and a current driver’s license
Nice To Haves
- Previous social services or behavioral health experience
- Bachelor’s degree in Social Services, Psychology, or related field
Responsibilities
- Provides targeted case management services as described by the Medicaid manual
- Supports clients in following individualized care plans and identifying case management goals
- Completes documentation on services provided within Valley’s standards
- Meets productivity expectations as assigned by supervisor
- Observes and collects client urine samples as necessary
- Transports clients to appointments as necessary
