About The Position
Support Growth, Recovery, And Success At Adult Outpatient Valley West , you'll help clients build the skills, confidence, and resources they need to thrive. As a Case Manager, you'll provide compassionate, goal-focused support while partnering with clients to promote recovery, independence, and long-term success.
Requirements
- Case Manager certification (post hire as needed - required within 120 days of start date)
- CPR certification (post hire as needed)
- Minimum age of 21 and a current driver’s license (for driving positions)
Nice To Haves
- Previous social services or behavioral health experience
- Bachelor’s degree in Social Services, Psychology, or related field
Responsibilities
- Provides psychosocial rehabilitative services as described by the Medicaid manual
- Supports clients in following individualized care plans and identifying case management goals
- Completes documentation on services provided within Valley’s standards
- Meets productivity expectations as assigned by supervisor
- Distributes medications as necessary
- Transports clients to appointments as necessary
